Services
Diagnostic Evaluation (90801)
  Personal interview with the patient lasting 45-60 minutes in most cases. The interview includes a history of the presenting illness as well as a detailed medical and psycho-social history. The interview ends with an initial diagnosis and formulation of a treatment plan.

Medication Management Session (90862)
  15 minute appointment that is problem focused. Medications are monitored and managed. Limited supportive therapy is also provided and there is coordination of care with the patient’s psychotherapist and/or other physicians.

Brief Psychotherapy Session (90804/90805)
  25 minute appointment for psychotherapy that may be combined with management of medication.
Available to patients paying out-of-pocket. †
Full Psychotherapy Session (90806/90807)
  45 minute appointment for psychotherapy that may be combined with management of medication.
Available to patients paying out-of-pocket. †
Forensic Evaluation
  Formal psychiatric evaluation that is performed in the service of addressing a legal issue. This is not covered by insurance. These evaluations require considerably more time and the preparation of a formal written report. Services are billed at an hourly rate. A retainer must be paid in advance.
Fitness for Duty Evaluation
  Formal psychiatric evaluation that is performed at the request of an employer/employee. The evaluation is used to determine if an employee has the mental and emotional capacity to perform a specific job or duty. These evaluations require considerably more time and the preparation of a formal written report. Services are billed at an hourly rate. A retainer must be paid in advance.

† Because most insurance plans to do not pay equitable benefits for more time intensive services, Dr. Van Doren does not accept insurance benefits for these services (90804, 90805, 90806, and 90807). Certain exceptions may apply based on insurance plan
