PAYMENT & CANCELLATION POLICIES
EMERGENCIES

Services are paid in full when the appointment is scheduled. Appointments may be canceled or rescheduled up to 48 hours prior to the appointment without penalty.   Charges for appointments canceled more than 48 hours in advance are 100% refunded to the credit card on file.  Charges for appointments canceled less than 48 hours in advance are NOT refunded and the entire fee is forfeited. 

LATE ARRIVALS

If you arrive late, Dr. Van Doren will see you for whatever time you have remaining. However, he cannot extend your session as other patients are waiting at their designated times.

In the event of a life-threatening emergency, call emergency services at 911 immediately. If you are in crisis and need help now call the National Suicide Prevention Lifeline at 1-800-273-8255.

PRESCRIPTIONS & CONTROLLED SUBSTANCES

Dr. Van Doren may prescribe medications for you depending on your symptoms, treatment plan, and health status.  

The DEA requires at least one "in-person" encounter with a patient before prescribing a controlled substance. Controlled substances include Adderall, Ritalin, Concerta, Provigil, Nuvigil, Vyvanse, Valium, Xanax, Ativan, clonazepam, muscle relaxants, opioids, and other similar medications. Therefore, patients taking controlled substances must initiate care with an office visit, subsequent visits may be via telepsychiatry.  See Title 21: Controlled Substances Act of 1970 and the Ryan Haight Act of 2008.

 

Dr. Van Doren uses electronic prescribing and can immediately send your prescriptions to any pharmacy. To request refills please contact Dr. Van Doren directly by email (info@drvandoren.com) or text (612.548.1502) and include your name, date of birth and the name of the medication.  Please allow 48 hours for refills. 

 
INSURANCE
HSA & FSA
 
 

Dr. Van Doren does not accept insurance.  You may pay for appointments directly from your HSA or FSA account.  If you have out-of-network benefits, you may request reimbursement from your insurance provider.  Please request a detailed statement from the doctor if you wish to file a claim.  

 

Please check with your insurance provider regarding policy and instructions for submitting your claims for reimbursement. 

For your convenience, we accept credit card payments at the time you schedule an appointment.   Payment with HSA and FSA debit cards accepted. 

Psychiatric care is a covered medical expense and you can use funds from your HSA or FSA account to pay for your appointment.   For your convenience, your HSA or FSA debit card is accepted at the time you schedule your appointent.
